You can fill up an application at the Crew Pursers Office and send your request for the program. Here is the thing, Cruise Lines give priority to the guests (which is normal) and if that particular cruise you book sells out your cruise will be canceled. So if you plan to take your family on a cruise I suggest you do it offseason when the cruise ships are not fully booked.

Applications will be confirmed on a first come, first served basis.
Offer is limited and subject to availability of selected sailings.
Once booking is confirmed and accepted, full payment is due within 72 hours & standard cancellation fee applies.
